Note: Many projects require a combination of different tiers.
1. TIDY UP EXISTING
$40/page
DESCRIPTION
Tidy up existing forms by adjusting layout and spacing and/or adding branding.
Is this tier for me?
This tier is good for forms that were made by a non-designer, missing a digital version, have an inconsistent design from page to page, or lack professional branding or identification.
2. UPDATE EXISTING
$50/page
DESCRIPTION
Tidy up existing forms as described in Tier 1, while also implementing minor changes or updates.
Is this tier for me?
This tier is good for forms that need minor changes, like adding/removing form fields or updating any wording.
3. CREATE NEW
Price varies
DESCRIPTION
Create a brand new form or educational handout from scratch.
Is this tier for me?
This tier is good for practices who are either just starting out, or have implemented a change to their practice that requires a patient form or educational handout.
Examples of an educational handout include how to use a medical device, when to call your doctor, discharge or procedural instructions, guide for at-home interventions, etc.
4. TOTAL REDESIGN
$80/page
DESCRIPTION
Redesign an existing form that requires changing the form’s layout and structure.
Is this tier for me?
This tier is good for existing forms that contain all of the necessary data fields, but are currently designed in a way that is confusing or unsightly. (If your patients often ask for clarification, this is a clear sign that a redesign is needed.)
5. LAB REQUISITION

DESCRIPTION
Create a new or redesign an existing lab requisition form.
Is this tier for me?
This tier is for labs who need to change the form fields on an existing requisition, struggle to use their current form due to spacing/layout/text size, etc., or need a new requisition designed from scratch.
6. DIGITAL FORM
Price varies
DESCRIPTION
Collaborate with the client to create a digital version of a form.
Is this tier for me?
This tier involves adding forms to the client’s website as either a PDF download, a fillable PDF, or an interactive webform.
Please note that some of these options require HIPAA-compliant security measures via a subscription-based third party that is not affiliated with Good Health Forms.
